Policy Overview:
Jumpstart 2 Recovery LLC offers a sliding fee scale based on family size and income. This scale determines the fee per session for eligible clients, ensuring services remain accessible. The sliding fee scale is applied uniformly to all clients who meet the eligibility criteria. No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule based on family size and income

Eligibility Requirements:
Clients must provide documentation of family size and income to determine eligibility for the sliding fee scale. Acceptable documentation includes but is not limited to:

  • Recent tax returns

  • Pay stubs (last 30 days)

  • Proof of unemployment benefits

  • Proof of enrollment in government assistance programs such as Medicaid, SNAP, or TANF

Clients choosing not to use insurance may access the sliding fee scale with proof of financial eligibility.

Basic Cash Rate:
For clients who opt not to use insurance, the basic cash rate is $100 per session. Sliding fee scale adjustments will reduce this rate for those who qualify.

Fee Determination:
The sliding fee scale is calculated based on the U.S. Department of Health and Human Services Federal Poverty Guidelines, updated annually. Discounts range from 10% to 90% of the basic cash rate, with specific brackets determined by income as a percentage of the Federal Poverty Level (FPL):
•    0–100% of FPL: $10 per session
•    101–150% of FPL: $25 per session
•    151–200% of FPL: $50 per session
•    201–250% of FPL: $75 per session
•    Above 250% of FPL: $100 per session

Client Agreement:
Clients approved for a sliding fee scale must sign the "Client Payment Plan Agreement Form" (modified for the sliding fee structure). This agreement will outline the cost per session, total amount owed, and payment terms. Failure to comply with payment arrangements may result in termination of services.

Signature

Annual Review:
Sliding fee scale eligibility must be re-evaluated annually or upon significant changes in family size or income. Clients must notify the practice immediately of such changes to update their eligibility status.

Confidentiality:
Information provided by clients for determining eligibility is confidential and will only be used for the purpose of establishing sliding fee scale eligibility.

If you believe you may qualify for our sliding fee scale, please contact our clinic to discuss eligibility. You can call us at 616-512-3210 or email us at admin@jumpstart2recovery.com to request the sliding fee discount. We will ask you to provide proof of household income (such as a recent tax return, W-2 form, or proof of government assistance) to determine your eligible fee reduction. Note: There is no online form for the sliding scale – please speak with our staff and submit your documentation directly to our office.
